United voice and networking opportunities
Our network provides feedback at local, regional, and provincial Primary Care Councils that will reflect the local North Toronto context and demographics. We create equal opportunities for primary care providers to engage with and benefit from service of the NT PCN (i.e., meetings, QI initiatives, committees, projects).
Collaborative work responding to crisis and improving practice
Together, we will address crisis and service delivery (i.e., Neighbourhood Care Team, cQIP: Preventative Screening, PAC Calls, COVID-19 response and recover, and winter surge planning).
Access to resources and team-based care
Our Network provides support in facilitating funding proposals to Ontario Health and the Ministry of Health. We improve access to specialists and interprofessional teams (i.e., SCOPE, TIP, PCT) to assist patients with timely, appropriate, and seamless care while supporting primary care providers (i.e., service redesign, QI initiatives, OHT Committees, projects).
