31 May Looking for funding to grow your primary care team?
Funding is available from the Ontario government to expand existing interprofessional primary care teams and/or create up to 18 new teams in communities with the greatest need. The goal is to connect more people with primary care closer to home, in communities across the province, especially those who are vulnerable and marginalized.
We are applying!
The North Toronto Primary Care Network and the North Toronto Ontario Health Team are working together to submit an application to allow us to help our community members who do not yet have access to primary care, especially those who are vulnerable and marginalized. The application will look to expand supports available to North Toronto primary care providers through existing assets, such as the Interprofessional Primary Care Team and SCOPE.
If you have ideas on what we should focus on to support the needs of unattached patients, please share your suggestions with us at north.subregion@sunnybrook.ca.

The North Toronto OHT will be happy to endorse the application of interested North Toronto primary care clinics whose application meets the following criteria:
- Full completion of Expression of Interest
- Demonstrated use of North Toronto data for development of Expression of Interest
- Plan included in Expression of Interest to address needs of unattached in North Toronto
- Evaluation plan included to monitor performance
Please request your endorsement letter by sending your completed Expression of Interest to north.subregion@sunnybrook.ca by Monday, June 12, 2023 at 5 p.m.
