Building capacity through collaboration

On a cold, snowy morning in January, Sue, Gian and Doris walk into a midtown Toronto long-term care (LTC) home, eager to start their weekly rounds.

This small but mighty team of registered nurses make up Sunnybrook’s Nurse-Led Outreach Team (NLOT) – a group dedicated to providing skilled, time-sensitive care to senior residents in five long-term care (LTC) homes across North Toronto.

The NLOT, a partnership between Sunnybrook and the North Toronto Ontario Health Team (NTOHT), is made up of health care organizations and providers dedicated to improving the coordination of primary care across North Toronto. The team was created to support LTC homes in response to the devastation caused during the COVID-19 pandemic and to offer integrated care between hospitals and community partners.

The primary mission of the NLOT is to enhance residents’ quality of life by minimizing unnecessary visits to the Emergency Department. With comprehensive in-home assessments and patient monitoring, the team provides timely access to urgent outpatient services, providing peace of mind for both residents and their families.

“Sunnybrook’s NLOT team has provided tremendous support to both residents and staff, ” says Monica Klein-Nouri, executive director, Meighen Health Centre. “Their timely assessment and in-home care ensure residents receive prompt, high-quality care without the need to leave the home.”
