14 Feb A roadmap for our first year

On January 31, 2020, key stakeholders came together for a planning day to guide the work of North Toronto Ontario Health Team. Six committees have been formed: governance, operations, digital, quality, communications and community engagement. Our first year will focus on seniors with high risk needs. These patients will typically have at least one complex illness, as well as other co-morbidities, and have physical care needs and psychosocial barriers. The North Toronto Ontario Health Team will also look at seniors with “rising risk needs” (conditions not under control and poised to become more high-risk).
