The North Toronto Ontario Health Team is working with its members to help residents in North Toronto find a family doctor or nurse practitioner.

Why should I have a family doctor or nurse practitioner instead of just using a walk-in clinic?
Walk-in clinics are helpful for one-time or urgent issues, but they don’t provide ongoing care. A family doctor or nurse practitioner:

Who is eligible to sign up for a family doctor or nurse practitioner?

I don’t live in North Toronto, but I work here. Can I still sign up?
Unfortunately, you must live in North Toronto to be eligible to sign up.
If you live elsewhere, you can still find a family doctor or nurse practitioner near you through Health Care Connect or by calling 811.
What if I already have a family doctor or nurse practitioner, but they are far from me?
If your current provider is very far away, you may still be eligible to sign up to be connected with a provider closer to where you live.
Are there any age limitations for who can sign up?
No, there are no age limitations. We accept individuals of all ages as long as they meet the eligibility criteria (live or work in North Toronto, have OHIP or IHIP, and do not currently have a primary care provider).
How do I sign up?

What information will I need to provide?
We will collect basic information about you to understand your needs. This helps us match you with a provider who is the right fit.
Why are you collecting this information?
The information you share allows our team to assess your care needs and connect you with the most appropriate provider based on your preferences.
I don’t want to share my health card information. Do I have to?
We understand that you may not feel comfortable sharing your health card information right away. Your information is kept confidential and only used to connect you to a primary care provider. Sharing it with us when completing the intake form makes the process faster and easier. If you prefer, you can complete the form without your health card number and provide it later when you are contacted.
I signed up. What happens next?
If you are eligible, one of our primary care partners will contact you to help complete your registration and connect you to a provider.
How long will it take to be connected to a provider?
The timeline depends on provider availability, but our partners will reach out as soon as possible once your intake form is processed.
Is there a cost to get connected?
No. Signing up and being connected to a family doctor or nurse practitioner is completely free.
Can I choose my doctor or nurse practitioner?
We do our best to match you with a provider who fits your care needs and preferences. While you may not be able to choose a specific provider, our team works to find the best available match for you.
What if I don’t like the doctor or nurse practitioner you matched me to?
We do our best to match you with a provider who fits your care needs and preferences. If you are not happy with the doctor or nurse practitioner you’ve been matched with, we will try our best to match you with another provider.
I need care while I am waiting to get attached. Where can I go?
If you need care while waiting to be connected with a family doctor or nurse practitioner, you can continue to use walk-in clinics, urgent care centres, or speak with a pharmacist for minor health concerns. In an emergency, always call 911 or go to the nearest emergency department.
